How the guidance works
Step 1 — Share your basics. Use the request form on our home page. Tell us you are interested in Oak Street Health Medicare or ACA guidance.
Step 2 — We clarify your coverage type. A representative can ask whether you have Medicare (including Part A/B or Advantage), an ACA plan, Medicaid, or another type—so the information matches your situation.
Step 3 — Learn about doctor options. You may hear whether participating providers are available near you, how scheduling might work, and what to bring to a first visit (ID, insurance card, medication list).

Is this only for seniors?
Medicare guidance is most often for people 65+ or those who qualify earlier due to disability. ACA guidance can apply to adults of many ages who bought coverage on the marketplace. Share your situation on the form and we will route your request appropriately.
Does this mean a specific doctor will accept me?
Availability depends on your plan, location, and provider participation. We help you understand the process; final acceptance is always confirmed by the office and your insurer.
